Required Forms

The following is a list for DO-IT-YOURSELFERS.

Keep in mind that if you meet with a member of MOVeteranAid.org and/or the American Association of Wartime Veterans and your veteran meets the qualification requirements, you can have all of this work done for you at no cost.

Collecting and filling these out on your own will not guarantee that your loved one will receive the benefit. Many applicants who try this without professional assistance fail.

• Discharge/Separation Papers (DD-214).  DON'T SEND YOUR ORIGINALS. Follow our instructions for obtaining the papers and you'll at least have a backup if these measures fail.

• Net Worth information, including bank accounts, CDs, Trusts, Stocks, Bonds, Annuities, etc.
• Proof of all income from pensions, retirement, interest income from investments, annuities, etc.
• Copy of Marriage Certificate and all marital information.
• Copy of the Death Certificate, if applicable.
• Proof of insurance premiums, medications, medical bills or any other medical expenses that are not reimbursed by insurance, Medicare, or Medicaid.
• Physician statement. This statement should include all current  diagnoses, medical status, prognosis, and statements about the potential claimant's  ability to care for self, ability to travel unattended, etc.
• List of all doctors and hospitals visited in the last year.
